Managing a Needlestick Injury in a Dental Practice

How Occupational Health Support Helped Protect Both Employee and Employer

A dental practice contacted David Barber Occupational Health following a needlestick injury involving a member of its clinical team.

Needlestick injuries are among the most concerning workplace incidents within healthcare environments. They can expose employees to blood-borne viruses such as Hepatitis B, Hepatitis C and HIV, creating anxiety for staff while placing significant responsibilities on employers to ensure appropriate medical assessment, follow-up monitoring and duty of care.

The Challenge

Following a routine dental procedure, a dental nurse sustained a needlestick injury while handling clinical instruments.

The incident was reported immediately in line with workplace procedures. While the risk level was initially unknown, the practice needed to ensure that the employee received prompt clinical assessment and that all appropriate occupational health protocols were followed.

Like many healthcare employers, the practice wanted reassurance that:
   • The employee's health would be protected.
   • Appropriate clinical investigations would be completed.
   • Follow-up monitoring would be managed correctly.
   • The organisation remained compliant with its health and safety obligations.
   • Staff wellbeing and confidence were maintained throughout the process.

Our Occupational Health Solution

The employee was advised to attend Accident & Emergency immediately for an urgent risk assessment.
Hospital clinicians reviewed the circumstances surrounding the exposure, including:
   • The nature of the injury.
   • The likelihood of blood-borne virus transmission.
   • The source patient's medical history where available.
   • The employee's Hepatitis B vaccination status.

Baseline blood tests were undertaken and the level of risk assessed. In situations where there is a significant risk of HIV exposure, Post-Exposure Prophylaxis (PEP) may be prescribed to reduce the likelihood of infection.

Following the hospital assessment, David Barber Occupational Health coordinated the employee's ongoing occupational health management programme.

Our Occupational Health Nurse arranged a structured follow-up schedule at our Derby clinic, including blood testing at:
   • 6 weeks
   • 12 weeks
   • 24 weeks

Monitoring included screening for:
   • Hepatitis B immunity status
   • Hepatitis C infection
   • HIV infection

Throughout the process, the employee received confidential support, clinical guidance and regular communication, helping to reduce anxiety while ensuring appropriate medical surveillance was maintained.

The Outcome

The employee completed all recommended follow-up testing, with results reviewed by our occupational health clinical team.
The structured monitoring programme provided reassurance to both the employee and employer that:
   • Appropriate clinical surveillance had been completed.
   • Potential health concerns would be identified quickly.
   • Workplace responsibilities had been fulfilled.
   • The employee could continue working with confidence.

By combining rapid clinical guidance, occupational health expertise and ongoing support, the practice was able to demonstrate a strong duty of care while protecting employee wellbeing and maintaining compliance with workplace health requirements.

Why Occupational Health Support Matters After a Needlestick Injury

Needlestick injuries can have significant physical, emotional and operational consequences for healthcare organisations.
Prompt occupational health involvement helps employers:
   • Protect employee wellbeing.
   • Meet health and safety obligations.
   • Manage workplace risk effectively.
   • Ensure appropriate medical monitoring is completed.
   • Reduce uncertainty for managers and staff.
